ELIGIBILITY CRITERIA
A. The young adult must be between the ages of 18 to 25.
B. The youth must have a mental health disorder diagnosable under the current edition of the Diagnostic and Statistical Manual of Mental Disorder published by the American Psychiatric Association. If the young adult does not have a diagnosis, but symptoms indicate a behavioral health disorder and criteria A, and C-G are met the young adult meets the criteria.
C. The young adult exhibits significant risk and needs in specific Life Domains.
D. The young adult is at risk of a more restrictive level of care;
E. The young adult is involved in the judicial system, homeless or unemployed.
F. The young adult is at-risk of committing a criminal offense; or at-risk of becoming homeless; or at risk of failure in the community.
G. Strong commitment and participation is agreed to and demonstrated.
There is no fee for service.
